New Patient Inquiries

Start with a free 15-min call

A free 15-minute phone conversation with Dr. Rai — a chance to talk about what's bringing you in and whether this practice is the right fit before you commit to becoming a patient. Submit the inquiry form below; once you do, you can either schedule your call directly from the calendar or wait for Dr. Rai to reach out within 1–2 business days.

Submit Inquiry & Schedule

Takes about a minute. Continue reading below if you'd like to know what to expect first.

How it works

Three short steps

1. Submit the inquiry form

Click the button at the bottom of this page to open the secure inquiry form. Share your contact information and a sentence or two about what's bringing you in — the whole form takes about a minute.

2. Choose how to schedule

After you submit, you'll have two options:

Schedule directly

Pick an available 15-minute time slot from Dr. Rai's calendar right after submitting. You'll get a confirmation email with the call-in details and a few notes on what to expect — no preparation needed on your end.

Wait to hear back

Submit without booking, and Dr. Rai will personally reach out within 1–2 business days to find a time that works. Useful if none of the immediate slots fit your schedule.

3. Have the call

Dr. Rai will call you at the scheduled time. The conversation is brief, informal, and not a clinical appointment — you won't be expected to share detailed personal history or complete questionnaires. If it's a good fit, he'll explain the next steps for the full intake form and your 90-to-120-minute initial evaluation. If it isn't a fit, he'll do his best to point you toward a colleague who may be a better match.

Before you submit

This practice serves adults 18 and older on a private-pay (out-of-network) basis. It's not the right setting for opioid use disorder requiring buprenorphine or methadone, acute detox, severe bipolar I or schizophrenia, active eating disorders, dementia, autism evaluations, or court-ordered treatment. If any of these is your situation, the inquiry call is still the right place to ask — Dr. Rai will help point you toward a colleague who's the right fit.

If you're in crisis right now, please call or text 988 or 911 instead of using this form.
