Fifteen-minute med checks don't fit a complicated life
The people who carry the most responsibility often get the thinnest psychiatric care: a rushed appointment every three months, a different prescriber than last year, a portal message answered by whoever is on shift. Meanwhile the actual problems — burnout that's tipping into depression, anxiety that's running the show in board meetings, insomnia that's been “temporary” for two years, a marriage strained by the same drive that built the career — need time, continuity, and a physician who can hold the whole picture.
Dr. Rai's practice was designed as the alternative. He is a board-certified psychiatrist trained at the University of Michigan, with a two-year fellowship in psychodynamic psychotherapy at the Michigan Psychoanalytic Institute. He provides both the medication management and the psychotherapy himself, in the same session, in one continuing relationship. There is no care team to coordinate and no one to repeat your story to.
What concierge means here
Direct phone and email access to Dr. Rai, with responses from him rather than from staff. Appointments at 7 a.m. before the market opens, in the evening after the last meeting, or on Saturday mornings. Sessions of 45, 60, or 90 minutes depending on what the work requires. In-person visits in a discreet, restored 1884 home in downtown Ann Arbor, or secure video when you're traveling. And same-week access when something urgent comes up, because a small practice can actually offer that.
Confidentiality you can rely on
Because this is a private-pay practice, no diagnosis, medication, or visit ever enters an insurance database. Nothing is discoverable by employers, boards, investors, or credentialing committees through a claims record, because no claims record exists. For physicians, attorneys, and other licensed professionals, that separation is often the deciding factor in seeking care at all. Records are released only with your explicit written consent, and Dr. Rai is glad to discuss the specifics before you begin.
The work usually starts with one of these
Burnout that rest doesn't fix. Anxiety that shows up as irritability, control, or 3 a.m. wakeups rather than worry. Depression that hides behind high function. ADHD that was manageable until the role outgrew the coping strategies. Drinking that has quietly moved from social to necessary. Grief, divorce, succession, the sale of a company, or the strange flatness that can follow finally getting everything you worked for.
Treatment combines careful, conservative prescribing with genuine psychotherapy. Many patients do their best work in 60- or 90-minute sessions every week or two during an active phase, then step down to maintenance visits once things are stable. Fees are listed openly on the fees page, and detailed superbills support out-of-network reimbursement, HSA, and FSA payment.
