The depression that surfaces at ninety days, or the insomnia that takes months to resolve. People in recovery know this terrain well, and so does anyone who works in it: the substance was rarely the whole story. Treating what's underneath is where relapse prevention and genuine wellbeing actually live.

Dr. Rai works with adults in every phase — early sobriety, decades of recovery, harm reduction, or considering change for the first time. Treatment combines careful medication management with psychotherapy that takes the function of the substance use seriously, in one continuing relationship with one physician.

What he offers, specifically

Naltrexone for alcohol use disorder, oral or injectable, when indicated. Nicotine cessation with varenicline, bupropion, NRT, and real behavioral support. Dual-diagnosis care for the depression, anxiety, PTSD, ADHD, and bipolar II that travel with substance use. Thoughtful prescribing throughout: no benzodiazepines where they'd undermine recovery, no stimulants without a careful evaluation, and a clear rationale for every decision. One honest boundary: Dr. Rai does not prescribe buprenorphine or methadone, so patients whose primary concern is opioid use disorder are warmly referred to specialized addiction medicine colleagues from the very first call.

For licensed professionals in recovery

Physicians, nurses, attorneys, pilots, and other licensed professionals navigating recovery alongside their careers receive particular discretion. Because this is a private-pay practice, there is no insurance claims trail, and nothing reaches employers, boards, or monitoring programs without your explicit written consent.

Washtenaw County has one of the best-developed recovery communities in the Midwest — Dawn Farm, Home of New Vision, Brighton Center for Recovery, U-M Addiction Treatment Services, and a deep network of AA, NA, SMART Recovery, and Refuge Recovery meetings. Dr. Rai sees his role as complementary to all of it. With your consent, he coordinates with sponsors, recovery coaches, IOP teams, and existing therapists, and he refers into that community as readily as he receives from it.

Asked before the first call

Alcohol, cannabis, stimulant, and nicotine use disorders — alongside any co-occurring depression, anxiety, ADHD, PTSD, or bipolar II (dual diagnosis is the standard here, not the exception). Medications used when indicated include naltrexone for alcohol use disorder and varenicline, nicotine replacement, or bupropion for nicotine cessation. The practice does not prescribe buprenorphine or methadone for opioid use disorder, and does not provide acute medical detox — both require specialized addiction-medicine networks Dr. Rai isn’t structured to provide. Yes — across the full spectrum, from people considering change for the first time to people years into recovery dealing with what sobriety alone didn’t resolve. Treatment may include naltrexone (oral or Vivitrol) when indicated, alongside psychotherapy and coordination with whatever recovery supports you’re using. No acute medical detox; patients needing supervised withdrawal are referred to a detox facility first and seen here afterward.

With patient consent, Dr. Rai coordinates directly with clinicians at Dawn Farm, Home of New Vision, Brighton Center for Recovery, U-M Addiction Treatment Services, and with sponsors, recovery coaches, and IOP teams. Most patients in formal programs use this practice for medication management and individual psychotherapy alongside their program’s group and peer-recovery work.
